Dr Vasco Nogueira is on night shift in the emergency department of Hospital de Santa Maria when a patient arrives at three in the morning. Male, fifty-two years old, name: Manuel Rodrigues. Complaints: chest pain, dizziness, paraesthesia in the left arm. Dr Vasco acts quickly — he orders an electrocardiogram, tests and continuous monitoring.

O Dr. Vasco olha para o doente na cama. Está vivo, monitores a biperem com regularidade, olhos fechados.

Dr Vasco closes the file and opens it again. It is not an illusion. In 1987, a Manuel Rodrigues with partially matching data was admitted to this same hospital with the same symptoms and died. Dr Vasco looks at the patient in the bed. He is alive, monitors beeping regularly, eyes closed.

Uma enfermeira entra com os resultados. O electrocardiograma confirma isquemia. Enquanto tratam o doente com urgência, o Dr. Vasco encontra tempo para fazer uma pesquisa rápida. A nurse enters with the results. The electrocardiogram confirms ischaemia. While they treat the patient urgently, Dr Vasco finds time for a quick search. The Manuel Rodrigues of 1987 was fifty-two years old. Today's Manuel Rodrigues's father had the same name. Hereditary cardiac condition: Brugada syndrome with family predisposition.

O Dr. Vasco estabiliza o doente e contacta a cardiologia. Enquanto espera o especialista, escreve uma nota no processo: "Antecedentes familiares de síndrome de Brugada. Verificar história paterna." Dr Vasco stabilises the patient and contacts cardiology. While waiting for the specialist, he writes a note in the file: "Family history of Brugada syndrome. Check paternal history." He then sits in an empty corridor and thinks about what it means to treat, at three in the morning, the same set of symptoms that a colleague treated thirty-seven years ago in this same building — and this time to save the patient.

A medicina, pensa ele, não é linear. É uma conversa entre o passado e o presente, conduzida por pessoas que muitas vezes não sabem que estão a dialogar com a história. Os doentes chegam com corpos que guardam memórias mais longas do que as suas próprias vidas. Tratar um doente é, às vezes, responder a uma pergunta que outra pessoa fez há décadas.

Medicine, he thinks, is not linear. It is a conversation between past and present, conducted by people who often do not know they are in dialogue with history. Patients arrive with bodies that carry memories longer than their own lives. To treat a patient is, sometimes, to answer a question that someone else asked decades ago.
